The Development of health management information system must compliant with the national guideline of health information exchange. This is essential to guarantee the consistency and efficient use of those medical data into medical information decisions. This paper propose a conceptual data model for ubiquitous healthcare management of pregnant lady that align with national guideline from Indonesian Ministry of Health and to ensure the continuity with the Indonesian referral system. In this research we propose the use of data modelling based on Fully Communication Oriented Information Modelling (FCO-IM) to ensure it consistency and efficient structure design of health data modelling, historical information regarding pregnancy condition and the proposed pregnancy management application. Based on the user acceptance test (UAT) of the proposed application system with test result 88,55%, we conclude that the conceptual data models are have high quality.
